INFORMED CONSENT FOR MINOR'S NUTRITION CARE Consent to Treatment and Treatment Disclaimers Nature of Services: The nutrition coaching services provided by ATHLETIC Fuel & ATHLETIC Coaching Hub may include personalised dietary guidance, meal planning, nutritional education and ongoing support aimed at improving overall health and wellness. Terms of Participation: I understand that as the parent/legal guardian, I am responsible for overseeing and ensuring the minor's participation in the nutrition coaching program. I acknowledge that ATHLETIC Fuel & ATHLETIC Coaching Hub will collect and use personal information about the minor solely for the purpose of providing nutrition coaching services. I agree to the terms and conditions outlined in the coaching agreement between ATHLETIC Fuel & ATHLETIC Coaching Hub and my child. Consent and Authorization: I hereby authorise my child to participate in the nutrition coaching services offered by ATHLETIC Fuel & ATHLETIC Coaching Hub and I consent to the collection, use, and disclosure of the minor's personal information as necessary for the provision of these services. Acknowledgment: By signing below, I acknowledge that I have read and understood the terms outlined in this parental consent form. I agree to allow my child to participate in the nutrition coaching program provided by ATHLETIC Fuel & ATHLETIC Coaching Hub. Payments and Refunds Payments for all services will be automatically charged to the clients nominated card or bank account after the session has concluded. Payments for coaching services are non-refundable. No-Show/Cancellation Policy In the event that the client is no longer able to attend a booked appointment the dietitian should be contacted to reschedule the appointment. Consultation calls cannot be cancelled or rescheduled within 12 hours of the appointment. Cancellations within 12 hours of the appoint will incur a charge of 50% of the consultation fee. This waiver outlines the terms under which Alpha Performance Group and Athletic Fuel School will collaborate to provide sports nutrition services to Alpha Performance Group’s athletes. Please read and check the box below.

Acknowledgment and Assumption of Risk: I, the undersigned, acknowledge that participation in sports nutrition programs involves certain inherent risks, including but not limited to, dietary changes, physical exertion, and potential health risks related to nutrition. I understand these risks and voluntarily choose to participate. Release of Liability: In consideration of being allowed to participate in the sports nutrition programs provided by Alpha Performance Group and Athletic Fuel School, I hereby release, waive, and discharge both parties, their affiliates, employees, and agents from any and all claims, liabilities, or damages arising from my participation in these services. Medical Disclaimer: I understand that the information provided by Alpha Performance Group and Athletic Fuel School is for educational purposes only and is not a substitute for professional medical advice. I affirm that I have consulted with a healthcare provider regarding any dietary changes or nutritional programs. Confidentiality: Both parties agree to maintain the confidentiality of any personal information shared during the nutrition program. No personal data will be disclosed without prior consent, except as required by law. Indemnification: I agree to indemnify and hold harmless Alpha Performance Group and Athletic Fuel School from any claims, actions, or damages arising out of my participation in the sports nutrition program. Governing Law: This waiver shall be governed by the laws of the State of Florida. Severability: If any provision of this waiver is found to be invalid or unenforceable, the remaining provisions shall continue in full force and effect. Acknowledgment of Understanding: I have read this waiver and fully understand its contents. I acknowledge that I am signing this waiver voluntarily and that it constitutes a binding agreement.
